Requesting an assessment from North East Lincolnshire Council

Anyone who appears to need care and support can ask, including a relative on their behalf. Request an assessment online or call adult social care on 0300 330 2870. Say clearly that you are asking for a care needs assessment under the Care Act 2014.

Adult social care assessments are carried out for the council by Focus Independent Adult Social Work through a Single Point of Access.

Inside a care needs assessment

The conversation is about daily life: dressing, a habitable home, getting about the home safely, washing, eating, staying in contact with people and managing the toilet. The assessor, typically a social worker or an occupational therapist, leads it, with relatives present if the person wishes.

Under the national criteria, needs qualify when they harm wellbeing significantly, are linked to an impairment or illness of body or mind, and leave them unable to manage at least two of the outcomes. Short-term support sometimes comes first: after an initial phone call, councils can provide aids or a spell of reablement, which is free for up to six weeks.

Getting ready for the assessment

  • Have the medicines list and diagnoses to hand, plus any falls or admissions.
  • A friend, a relative or, where nobody is available, an independent advocate can support you.
  • Explain what goes wrong on difficult days. Pride often leads people to say they cope.
  • For seven days beforehand, note each difficulty, when it happened, and the result.
  • Ask for a copy of the written assessment and care and support plan.
