NHS CHC Checklist: what it means
A screening tool, completed by a health or social care professional, to decide whether someone needs a full CHC assessment.
It covers 11 areas of need, each scored A, B or C. A full assessment follows if, for example, two areas score A, or five score B, or any of the starred areas (breathing, behaviour, drug therapies, altered states of consciousness) scores A.
